Brief summary describing the background and objectives of the trial Mediterraneandiet (MD) is a well-known pattern of diet full of antioxidants and probably has an anti-inflammatory effects.This study designed to evaluate tolerability, safety and potential efficacy of MD in children and adolescent patients with irritable bowel syndrome (IBS).

Experimental Group mediterranean diet group they will follow mediterranean diet for 6 months 6 months 40 children with IBS will be included and they will receive Mediterranean Diet for 6 months beside their regular treatment. Initial evaluation of all subjects will include history, physical examination and laboratory tests such as CBC, CRP, ESR, albumin, serum calcium, random blood sugar level, renal and hepatic functions, serum proteins, urine, and stool analysis. Fecal calprotectin in stool was measured in all included patients to exclude patients with inflammatory bowel disease. Follow up visits will be at 1, 3 and 6 months. IBS symptoms severity score (IBS‑SSS) questionnaire as well as IBS quality of life (IBS‑QoL) questionnaire will be also evaluated at start and at the end of our study. 40
Control Group none regular diet and regular treatment 6 months 40 children with IBS will be included and they will be on their regular diet and regular treatment with no dietetic intervention for 6 months. IBS patients aged less than 18 years diagnosed with ROME IV criteria for diagnosis of childhood irritable bowel syndrome : recent change in IBS therapy, gastrointestinal infection, history of gut surgery or radiation, celiac disease, overweight or underweight according to the centile curve,[2] chronic disease such asrenal failure or diabetes mellitus, and patients not adherent to our diet protocol.
