This study is a cluster randomized control trial that provide depression and anxiety counseling, and adherence education intervention package to adult tuberculosis patients on treatment for at least one month prior to the study in Addis Ababa, Ethiopia based on Health Belief Model (HBM). Health behavior theory is important to plan effective interventions to achieve changes in risky health behaviors including treatment non-adherence. The aim of this study is to evaluate the impact of psychological counseling and adherence education intervention on tuberculosis treatment adherence based on six HBM domains.
Thirty health centers will be selected from 53 health centers found in Addis Ababa by cluster sampling method. Selected health centers will be randomly allocated into control and intervention groups by simple random method. For each intervention and control group, 342 TB patients will be enrolled consecutively at selected health centers. Depression and anxiety counseling, and adherence education interventions will be provided for intervention group for four total months, once in a week for the first month of enrollment, and once in a month until the end of TB treatment. Daily time regimen of intervention provision will be 30 minutes based on the experience obtained from study reported from Bangladesh. In control group, the normal directly observed therapy short course strategy remains unchanged. Proportion of non-adherence, the impact of intervention and the mean difference on six HBM domains will be assessed using multiple logistic regression.
