Official scientific title Evaluating the implementation of routine antenatal US for estimating date of delivery in Malawi and Zambia; an interrupted time series trial
Brief summary describing the background and objectives of the trial A major barrier to effective prevention and management of preterm birth in Malawi and Zambia is the lack of an accurate, consistently applied, estimated date of delivery (EDD) for each pregnant woman. This is best achieved through ultrasound Scanning (USS) measurements prior to 24 weeks gestation. Indeed, the World Health Organisation (WHO) endorses the use of one ultrasound scan before 24 weeks gestation for all pregnant women. Such a goal is unattainable in many Low and Middle Income Countries (LMIC) due to lack of availability of trained staff and ultrasound machines. In participating health facilities in Malawi and Zambia we aim to provide an accurate, consistently applied estimated date of delivery (EDD) for each pregnant woman, which is maintained throughout the duration of the pregnancy and used to determine gestational age. We will assess the acceptability and feasibility of implementation of ultrasound scanning, and find ways to ensure gestational age are determined by a fixed EDD as part of routine early antenatal care. We will evaluate the process outcomes associated with this, including assessing adoption, fidelity and penetration, whilst reviewing the number of women who had an EDD established in early pregnancy and of those the number for whom this was not changed. We will also evaluate clinical outcomes of relevance including the rates of referral for pre-term, post-dates and the number of pregnancies where multiple births are diagnosed at the time of delivery. We will also evaluate stakeholders’ perceptions of the interventions and their implementation and review overall client satisfaction.

Experimental Group Ultrasound and Estimated Gestational Age Training 9 Months This is a prospective study that will implement a package of care to improve estimation of gestational age at health facilities and district hospitals in Malawi and Zambia. Quantitative data will be collected pre-intervention for a 3-month period, the intervention will be introduced at each site and implemented for six months, there will be a brief “wash-out’ period of one month and then a further three months of data collection. 6000

Service providers (including obstetricians, clinicians, midwives and maternity based support staff) in a participating facility. All women attending for antenatal care at any of the participating sites will automatically be enrolled into the study and excluded only if they do not wish to participate. Women who do not wish to participate. In this situation they would proceed with routine antenatal care without having an ultrasound scan. They may however, still be unavoidably exposed to educational materials. Their pregnancy data would not be collected. Service providers who do not wish to participate. In this situation we would encourage them to engage for a short interview to explain the reasons for their reluctance, allowing us to better understand and address barriers to implementation.
