Brief summary describing the background and objectives of the trial The Sutureless technique is a relatively new practice of nonclosure of flap after an impacted wisdom tooth removal. Compared to the complete closure of flap, the sutureless technique is hypothesized to have less severity of inflammatory sequelae that characterizes the few days after a surgical extraction. The lesser inflammatory sequelae is believed to also have an improved quality of life in the sutureless group

Experimental Group Sutureless non closure. no stitches placed one week this is a newer practice for impacted wisdom teeth extraction. Upon raising a 3-sided mucoperiosteal flap and completion of tooth extraction, the flap is simply repositioned without placement of suturesl. the oral mucosa wound is expected to heal by secondary intention. 36
Control Group complete closure 3 or more stitches one week this is the standard technique used. Upon raising a 3-sided mucoperiosteal flap and complation of tooth extraction, the flap is repositioned and multiple stitches placed to ensure a hermetic seal. the oral mucosa wound is expected to heal by primary intention 38 Historical

1. ASA I patients at least age 18years with an indication for extraction of an impacted MTM under local anaesthesia. 2. Patients with mesioangular, distoangular, horizontal and vertical impactions with a difficulty index of 3–8, according to Pederson’s criteria. 1. Patients who do not provide informed consent 2. Patients with difficulty index greater than 8 according to Pederson criteria 3. Patients for M3 surgery not requiring a full thickness mucoperiosteal flap or bone removal. 4. Presence of symptoms such as severe pain, facial swelling or limited mouth opening from any cause within 10 days preceding surgery. 5. Patients who have allergy or cannot tolerate the local anaesthetic agent, and the study drugs. This includes patients with peptic ulcer disease. 6. Patients on medications that could interfere with healing after surgery – steroids, oral contraceptives, other anti-inflammatory drugs for other reasons 7.
