TRIAL DESCRIPTION
Public title Evaluation Of The Effect Of Synthetic Exosomes On Periorbital Rejuvenation: A Single-Arm Clinical Trial
Official scientific title Evaluation Of The Effect Of Synthetic Exosomes On Periorbital Rejuvenation: A Single-Arm Clinical Trial
Brief summary describing the background and objectives of the trial Background and Rationale: The eyes are the aesthetic centerpiece of the face. As prominent facial features, the periorbital area is one of the first regions to reveal signs of aging and plays an important role in overall facial appearance. Aging skin is characterized by the appearance of fine and coarse wrinkles, rough and uneven texture, dryness, skin laxity and changes in pigmentation. ( Glaser et al, 2010) While aging is inevitable, its visible manifestations can be delayed or minimized. Regenerative medicine is a developing field focused on the repair, rejuvenation, replacement, or regeneration of tissues and organs to reestablish normal function ( Jafarzadeh et al., 2024) There is growing interest in therapies based on biological mechanisms of intercellular communication, which can modulate the inflammatory response, stimulate collagen synthesis, and promote skin repair. In this context, extracellular vesicles (EVs) playing a crucial role in cell-to-cell signaling deserve special attention. (Buzas et al,2023) (Sun et al,2024) Exosomes can be used as a delivery system for drugs, proteins, mRNA, miRNA, IncRNA, and small molecules to recipient cells, which can regulate macrophages, fibroblasts, vascular endothelial cells, and myofibroblast functions in wounds to promote healing (Bunggulawa et al., 2018). Exosomes are involved in regulating the proliferation of epidermal cells, fibroblasts, restoring the ECM components, lowering ROS levels and ultimately reversing skin aging. ( Wu et al.,2022 ) Objectives: To evaluate the efficacy of synthetic exosomes in the rejuvenation of periorbital region by assessing the improvement of skin texture, elasticity, dyschromia and wrinkle reduction.

INTERVENTIONS
Intervention type Intervention name Dose Duration Intervention description Group size Nature of control
Experimental Group Synthetic Exosomes Injection of 2 mL of solution per session, composed of 1 mL synthetic exosomes mixed with 1 mL of 0.9% sodium chloride (NaCI) solution. The mixture is equally distributed, with 1 mL administered to each periorbital area. 18 weeks duration Injection of 2 mL of solution per session, composed of 1 mL synthetic exosomes mixed with 1 mL of 0.9% sodium chloride (NaCI) solution. The mixture is equally distributed, with 1 mL administered to each periorbital area. One session every 2 weeks for 3 consecutive sessions. Patients are followed up at 2, 4, 6, and 18 weeks after the initial injection session. During each follow-up, assessments include serial standardized photography, evaluation using the Merz Periorbital Scale, and Global Aesthetic Improvement Scale (GAIS). 36
