Brief summary describing the background and objectives of the trial Background: Anterior cruciate ligament (ACL) reconstruction is frequently associated with moderate-to-severe postoperative pain that may hinder early mobilization and recovery. Motor-sparing regional techniques have gained increasing attention as part of multimodal analgesia. This study aimed to compare the analgesic efficacy of adductor canal block (ACB), infiltration between the popliteal artery and capsule of the knee (IPACK), and genicular nerve block (GNB) in patients undergoing ACL reconstruction.

Experimental Group IPACK group 20 ml solution of 0.25% bupivacaine 24 postoperative hours After GA induction, infiltration under ultrasonic guidance between the popliteal artery and femur. The infiltration will commence 2 cm distal to the popliteal artery and will be gradually withdrawn while infiltrating proximally. 20
Experimental Group GNB group 20 ml of 0.25% bupivacaine 24 postoperative hours After GA induction, ultrasound-guided infiltration targets the superomedial, superolateral, and inferomedial genicular nerves 20

ASA physical status I-II, of both sexes, aged 18-60 years, Scheduled for ACL reconstruction with a Hamstring graft from the same limb • Peroneous longus graft or adductor policis graft from the same limb. • Any type of graft from the other limb. • Severe cardiovascular diseases. • Significant neurological or psychiatric disorders • Patients with a history of coagulopathy or bleeding disorder. • Allergic to local anesthetics. • Injection site infection. • History of drug addiction. • Chronic opioid use. • Pregnant or suspected pregnancy Adult: 18 Year(s)-44 Year(s),Middle Aged: 45 Year(s)-64 Year(s) 18 Year(s) 60 Year(s) Both
