TRIAL DESCRIPTION
Public title A double-blind, randomised, placebo-controlled trial evaluating the efficacy and safety of oral nerandomilast treatment in patients with systemic sclerosis (SSc)
Official scientific title A double-blind, randomised, placebo-controlled trial evaluating the efficacy and safety of oral nerandomilast treatment in patients with systemic sclerosis (SSc)
Brief summary describing the background and objectives of the trial Systemic sclerosis (SSc) is a rare, heterogenous chronic immunemediated connective tissue disease with multi-system organ involvement and high mortality. The pathophysiology is a complex interplay of vasculopathy, immune dysregulation, and fibrosis. Currently, there are no approved drugs that fully address the pathogenesis of the disease or modify the disease course. Rituximab is approved for the treatment of SSc in Japan only, based on a Japanese Phase II investigator-initiated study in 56 patients with SSc funded by the Japan Agency for Medical Research and Development (AMED). For treatment of selected patients with severe disease and poor prognosis, in the absence of advanced cardiorespiratory involvement, high-intensity immunosuppression (usually including cyclophosphamide) followed by autologous hematopoietic stem cell transplantation (HSCT) may be considered. Preclinical studies show that PDE4B inhibition with nerandomilast has the potential to target the 3 main components of SSc pathogenesis: inflammation, fibrosis, and vasculopathy. The mechanisms demonstrated in preclinical data have translated into beneficial effects in Phase II and III clinical trials in patients with idiopathic pulmonary fibrosis (IPF) and progressive pulmonary fibrosis (PPF). The objective of the trial is to investigate the efficacy and safety of nerandomilast in patients with SSc at an oral dose of 18 mg bid compared to placebo on top of standard of care (SoC). The dose may be reduced to 9 mg bid temporarily or permanently based on individual tolerability or if used concomitantly with strong CYP3A (Cytochrome P450, isoform 3A (4)) inhibitors. No reduction of the IMP dose to 9 mg bid is allowed in participants while receiving concomitant moderate or strong CYP3A inducers. The primary objective is to demonstrate the superiority of nerandomilast for the hazard ratio for the time to all-cause death or disease progression. Disease progression is defined as adjudicated SSc-related clinically meaningful disease progression or complications or confirmed absolute decline from baseline in Forced vital capacity (FVC) % predicted ≥5% (for patients with ILD at baseline), or mRSS absolute increase ≥5 points and relative increase from baseline ≥25%.

INTERVENTIONS
Intervention type Intervention name Dose Duration Intervention description Group size Nature of control
Control Group Nerandomilast Nerandomilast 18 mg (bid); nerandomilast 9 mg (bid; to be used based on individual tolerability or during concomitant use of strong CYP3A inhibitors); oral All randomised trial participants will remain in the trial until the planned number of 283 primary endpoint events has been reached and the last randomised patient on trial treatment completes at least 52 weeks of treatment. Nerandomilast (BI 1015550), a selective inhibitor of the phosphodiesterase 4B (PDE4B), is in development for the indication of progressive fibrosing interstitial lung diseases (PF-ILD) including idiopathic pulmonary fibrosis (IPF) as well as for the indications systemic sclerosis (SSc) and Myositis. Based on the knowledge gained in the course of clinical and pharmaceutical development, the Formulation C1 and C2 have been developed as film-coated immediate release (IR) tablet at dose strengths of 9 mg, and 18 mg. Both trial formulations were intended to supply pivotal clinical trials. In addition, 6 mg dose strength (Formulation C1) was developed but never introduced in phase III clinical trials. 1 Active-Treatment of Control Group
Experimental Group Placebo Matching; oral - Matching placebo to BI 1015550 film-coated tablets consist of standard placebo tablet cores which are coated with the same film-coat as the active tablets All randomised trial participants will remain in the trial until the planned number of 283 primary endpoint events has been reached and the last randomised patient on trial treatment completes at least 52 weeks of treatment. Placebo to nerandomilast (BI 1015550) film-coated tablets 9 mg are light yellow, oval, biconvex, and about 9.5 x 4.6 mm in length and width. One side is debossed with the BI company symbol, and the other side is debossed with ‘F9’. Placebo to nerandomilast (BI 1015550) film-coated tablets 18 mg are light red, oval, biconvex, and about 12 x 5.9 mm in length and width. One side is debossed with the BI company symbol, and the other side is debossed with ‘F18’. There is no active pharmaceutical ingredient in the placebo formulation. Matching placebo to BI 1015550 film-coated tablets consist of standard placebo tablet cores which are coated with the same film-coat as the active tablets. 1

OUTCOMES
Type of outcome Outcome Timepoint(s) at which outcome measured
Primary Outcome The objective of the trial is to investigate the efficacy and safety of nerandomilast in patients with SSc at an oral dose of 18 mg bid compared to placebo on top of standard of care (SoC). The dose may be reduced to 9 mg bid temporarily or permanently based on individual tolerability or if used concomitantly with strong CYP3A inhibitors. No reduction of the IMP dose to 9 mg bid is allowed in participants while receiving concomitant moderate or strong CYP3A inducers. The primary objective is to demonstrate superiority of nerandomilast for the hazard ratio for time to all-cause death or disease progression. Disease progression is defined as adjudicated SSc-related clinically meaningful disease progression or complications (see below for definition), or confirmed absolute decline (see below for definition) from baseline in FVC % predicted =5% (for patients with ILD at baseline), or modified Rodnan Skin Score (mRSS) increase =5 points and relative increase from baseline =25%. Treatment comparison in the primary objective will be as randomised (per ITT [intention-totreat] principle), including the effect of any changes of the investigational treatment and the effect of initiation of new medications or changes in background medications during the trial. 52 Weeks
Secondary Outcome The secondary objectives are to demonstrate superiority of nerandomilast for the mean change from baseline in mRSS at Week 52, for the mean change from baseline in HAQ-DI at Week 52, for the mean change from baseline in FVC [mL] at Week 52, for the response rate difference in revised composite response index (rCRISS-25) at Week 52, for hazard ratio of time to each individual component of the primary composite endpoint, and for the mean change from baseline in ScleroID and digital ulcer total burden at Week 52. Treatment comparison in the secondary objective will be as randomised (per ITT principle), including the effect of any changes of the investigational treatment and the effect of initiation of new medications or changes in background medications until the planned visit at Week 52. Death will be handled by a composite analysis strategy. 52 Weeks
