TRIAL DESCRIPTION
Public title Long-acting User-friendly Novel Approaches for HMB management.
Official scientific title Phase 1 (placebo only) cross-over trial to assess acceptability, safety, and usability of two prototype placebo vaginal films among women with and without heavy menstrual bleeding in South Africa.
Brief summary describing the background and objectives of the trial Heavy Menstrual Bleeding (HMB) affects about 20% of women worldwide, with even higher rates in low and middle income countries. It is defined as menstrual blood loss over 80 mL per cycle or bleeding lasting more than seven days. HMB has significant physical, emotional, and social effects on the affected individuals. Levonorgestrel (LNG), a synthetic progestogen used in emergency contraception and in hormonal IUDs which are approved for treating HMB. These IUDs work by continuously releasing LNG to thin the uterine lining, alter blood vessel structure, reduce menstrual flow, and decrease uterine contractions providing both therapeutic and contraceptive benefits. However, hormonal IUD use in LMICs remains low due to cost, access challenges, myths, provider dependency, and concerns about safety and reversibility. Vaginal films offer an alternative: they are low cost, discreet, self administered, and do not require healthcare provider involvement or special storage. A one month LNG vaginal film could therefore be a promising option for African women seeking both HMB management and pregnancy prevention. The study described aims to develop and evaluate an LNG vaginal film as an accessible self care product for women in LMICs. Initial work will test placebo films in South African women to assess acceptability and safety. Findings from usability testing, lab data, and clinical insights will inform selection of a lead formulation for further development. Overall, the project integrates pharmaceutical and socio behavioral research to build evidence for a new self administered product that could expand contraceptive and menstrual health options for underserved populations.

Intervention type Intervention name Dose Duration Intervention description Group size Nature of control
Experimental Group LUNA Placebo Vaginal Film A The products being used in this study are the placebo prototype vaginal films A and B. The two films are soft and flexible with rounded edges and differ from each other by different mechanical properties and mucoadhesion. Participants will be randomized (1:1) to use two placebo films in either sequence A to B or B to A. Film use will occur in sequential order and will involve a single application of each film and monitoring for one month with a non-use period of approximately 7 to 14 days (1-2 weeks) in between. Films should not be inserted during menses. Film use will occur in sequential order and will involve a single application of each film and monitoring for one month with a non-use period of approximately 7 to 14 days (1-2 weeks) in between. Appearance is Flat, flexible, translucent to off-white film, with 110-150 μm thickness. Weighs 350-430 mg, with not more than 15% w/w water content. Microbial testing is not more than 100 CFU/g, Total Aerobic Microbial Count Not more than 10 CFU/g, Total Yeast/Mold Counts. Absence of the following: 1. Pseudomonas aeruginosa 2. Staphylococcus aureus 3. Candida albicans 20
Experimental Group LUNA Placebo Vaginal Film B Participants will be randomized (1:1) to use two placebo films in either sequence A to B or B to A. Film use will occur in sequential order and will involve a single application of each film and monitoring for one month with a non-use period of approximately 7 to 14 days (1-2 weeks) in between. 1 month Appearance is Flat, flexible, translucent to off-white film, with 110-150 μm thickness. Weighs 326-440 mg, with not more than 15% w/w water content. Microbial testing is not more than 100 CFU/g, Total Aerobic Microbial Count Not more than 10 CFU/g, Total Yeast/Mold Counts.
